Hi... It seems to be a clear case of severe health anxiety. I believe you have been going through a tough phase in your life for a long time and over the years you have been repressing your core issues from your conscious mind to your unconscious/subconscious mind. Now these issues may not be in your immediate awareness but are impacting your mental well-being and coming out in the form of such symptoms. Please consult a Psychiatrist and a Psychologist. Treatment of such issues consists of a combination of medication and psychotherapy both. You have taken SSRI's and antianxiety medicines but they need to be complemented by psychotherapy/counselling as well. Otherwise the core issues will not resolve and the symptoms will keep coming back as and when your brain becomes use to the neurochemical change brought by those medications. In some cases patients report feeling better with medications but symptoms coming back when they stop taking the medication. Please understand this, medication is meat to neurochemically address the imbalance cased by a condition (depression, anxiety, psychosis, etc.) but if the root cause is psychosocial than the treatment will take place when those core issues are addressed using psychotherapy/counselling. Mediation in such cases play a temporary role.
Next Steps
Consult a Psychiatrist and a Psychologist. CBT along with Lifestyle Changes and Interpersonal Guidance & Counselling is required. I know some good Psychiatrists in Bangalore and I can share contacts.
