Tremendous pain near neck

Hello sir/ma'am, I have tremendous pain near to my neck and shoulder. It makes me feel that my bone wants to expand. Because of which i feel a bad pain in my lower back head. It also make me feel to vomit. Please advise which doctor can help me. Do i have to show this problem to Orthopedic doctor.
