Treatment for lowering Creatinine anaemi

My father, age 54 years, having hypertension, type 2 diabetes and retinopathy has been diagnosed with Chronic kidney disorder in Feb'24 after he accidentally fell due to slippery surface. The doctor recommended blood test which revealed that his creatinine level was 2.4.After taking Medications, the level descreased to 1.58.but again the level increased to 2.7 along with haemoglobin content being too less i.e. 7.i want to ask what is the medications to use and how can we lower the creatinine levels and subsequently the anaemic conditions. Also the eye is having spots which is becoming a problem to focus on an object. What can we do?

His sugar needs to be well controlled and he needs to avoid nephrotoxic medications He may need to take erythropoitin injections for anemia. In addition,  he will need to undergo tests to assess iron and vitamin b12 status Pls consult an ophthalmologist for the eye issues
