Instead of extraction ask if there’s any way your premolar can be saved. Antibiotics and painkillers will take care of your severe pain, infection and swelling. If the tooth is badly decayed you might need an RCT followed by a post(screw) which supports the tooth filling , in cases , where there is no healthy tooth wall to support a filling. This can be followed by a crown so that your tooth is well protected from all sides. However if the infection is extensive and there is bone loss around the tooth, extraction will be the best option. Once the bone heals post extraction , you can go for a bridge or implant , to replace your extracted tooth.
