Hi, I am Dr. Atul Kumar Nimawat MS ENT
From your description, it seems you have been experiencing bilateral tinnitus (ringing in the ears) along with a sensation of ear fullness for the past four months, which started after an episode of severe cold. Your Pure Tone Audiometry (PTA) report being normal is reassuring, but persistent symptoms point towards Eustachian Tube Dysfunction (ETD), which can occur due to inadequate ventilation of the middle ear. Since nasal sprays have not provided much relief, further evaluation may be required. This may include an impedance audiometry (tympanometry) to assess middle ear pressure, nasal endoscopy to check for anatomical causes like adenoid hypertrophy or nasal allergies, and sometimes imaging if indicated. Tinnitus itself can also be multifactorial, and if the Eustachian tube problem persists, it can aggravate the ringing sensation.
