TPO positivity suggests auto immunity and you may require life long thyroxine treatment. TSH is still on higher side even with 50mcg then you need to increase dose. It takes 6 to 8 weeks for TSH to reflect change after increasing thyroxine dose. Consult any diabetologist or endocrinologist for dose increament
