Please say what is this and tell me the whether it is a serious condition or not.. and tell me whether I want to take any tablets or not because it is present for above 1 year it is not paining but I'm doing self check daily and I'm in depression please tell me elaborately and I'm a smoker

This is granulations basically pharyngitis You should take antibiotic course And yes smoking can be a cause

Your posterior pharyngeal wall appears granular and congested It is due to reflux pharyngitis and due to your smoking habit its worsening … Nothing very serious so dont worry You will have to work on your diet , quit smoking and take anti reflux medications
