TSH 5.99 with normal T3/T4 is subclinical hypothyroidism - it can explain the fatigue and low mood, but it does not explain 8 kg loss in 8 months. Unintentional weight loss of that size needs a cause hunt, not a weight-gain tonic. Get TSH repeated with anti-TPO after 6 weeks, plus CBC, ESR, blood

HbA1c, coeliac serology (tTG-IgA), stool routine and a chest X-ray. Long-standing IBS with weight loss and incomplete evacuation makes me want to rule out coeliac disease and
