Swelling in legs, sharp pain under knee

Hello Doctors, My mother is 49 years old and she is obese having weight around the 80kg. She has knee osteoarthritis issue on both for a long time and takes calcium, vitamins related to bones. She feels immensely sharp pain under the knees and in calf muscles. Whenever i see the calf muscles, I don't see any bulging veins outside but during oil massage or touching, I can feel bulging veins or too many swollen veins inside the calf muscles of both legs which are painful on compression or pressing. Are those varicose veins or are they normal to have ? I doubt whether she has varicose veins causing sharp pain while movements or standing for some minutes. I would like to know whether physiotherapy can help with this pain if it's varicose veins? Could you please give us any advice or treatment plans or suggestions on what we should ?  I have attached a picture as well. Thanks.

Get a venous Doppler both legs to see for varicose veins
