Suggestion for heart surgery

Hello Doctor, My brother complained about chest pain. We consulted the doctor, he advised to do CT Angiography. I have attached the report. Can you please let's know if there is any blockage? Does he need to undergo any heart surgery? We would really appreciate your valuable guidance. Thanks, Fani Bhushan

There are significant blockages, need to do conventional angiography as that is the gold standard, treatment is also decided with conventional angiography only not by CT angio
