* Heart problem / atypical cardiac ischemia
* Delirium from infection, dehydration, electrolyte imbalance, urinary infection, low oxygen, etc.
* Early neurological event (TIA/stroke)
* Medication side effect
* Hypoglycemia
The arm pain being previously labeled “bone pain” does not completely rule out a cardiac cause, especially if the symptoms are new or worsening.

Go to the ER / seek urgent medical care immediately if any of these occur:
* Chest discomfort or pressure
* Sweating
* Shortness of breath
* Increasing confusion
* Slurred speech
* Facial deviation or weakness
* Fainting
* Palpitations
* Severe agitation
* Reduced responsiveness
