So I developed this sudden chest pain(left side) where even if a move a bit or if my move a arm a bit ,it hurts like hell. The very last time I just stretched my upper arms to back.my left side chest paining all of  sudden, after diclofenac spray it was relieved. This has been occurring since 2 months.but now it has worsened. Is it muscle related pain or any underlying heart disease? Do I need to get my ecg done for this?

Once go through ECG. If it's normal, consult with orthopedic doctor.

Do ECG and check your BP.

As per the information the chest & arm pain must be due to muscle sprain, at this age Cardio vascular problems chances are bit less, but for your satisfaction check BP if it is within normal ranges & if there is no shortness of breath or lethargy then you can totally avoid ECG & other test for heart disease.
Next Steps
Check for BP, then consult an orthopedician.
