Features are suggestive of pressure injury (barotrauma) to Eustachian tube and PNS due to flying.
Ear examination is required to rule out middle-ear injury ( hemotympanum..)
Start short-course of
-N/d xylometazoline 2 drops TDS 3-5 days
-N/s fluticasone 2 puffs BD 1-2 weeks
-Steam inhalatio TDS
-Tab sinarest 1 tab SOS ( max to TDS)
- Gentle valsalva maneuver TDS
-If fever/purulent discharge persists, evaluate for acute sinusitis and need for antibiotics.
-Urgent ENT review if hearing loss, vertigo, or severe persistent pain develops.
Next Steps
Follow the above treatment for 5 days.
If symptoms settle, continue the remaining medications for another 5 days
except Xylometazoline nasal spray,
which should not be used beyond 5 days.

-Avoid flying during URTI / allergic episode
-Decongestant 30 min before takeoff/landing
-Chewing gum/sipping water during descent
