Son doesn't understand basic thing

My 5 year old son is very active. He is good in study and normal in other things but he doesn't understand basic things like how to react, doesn't follow instructions most of the time. He gets irritated and he lives in his world, wants to watch screens all the time and when we stop, he gets irritated and starts throwing tantrums . He runs behind other boys and gets irritated when they don't play with him and start crying. Don't know what to do
