As you had a left gluteal abcess one year ago and you have not taken any antibiotics then ,there is more chances of a dormant infection. The bacterial infection may be there in a low level. You are having discomfort in your Sacro illeac region.      Septic Arthitis is involvement of a joint. Excruciating pain will be there with. fever. You may be immobilised with pain No movement of joint.       In your case a complete haemogram to rule out infection , X-Ray of Pelvis ,CT of the painful region may help to pinch diagnosis

The possibility is there but we need to get blood work up done as well as Xray & MRI of the region. On co relating clinical examination and investigation, it can be told whether the joints are involved or not.
Next Steps
Get the investigations done as soon as possible.
