Need help to go through the attached report and advice me accordingly.that would be very helpful to me.

Your semen analysis report shows  about 15 millions sperm count,  this is very low count, other reports are not very clear. You should take appointment or consult privately through practo for discussion and ayurvedic treatments for this problem.
