You need to control your bp need to add some medicines. ECG is not bad according to the age. There is fluid retention in your body that's why there is swelling in your feet. Need to start diuretic which will reduce the swelling, reduce the blood pressure and reduce the anxiety. Can be due to gastritis so you need to add ppi also. You blood glucose is high so need further tests any many need to increase the dose and you need to fallow diabetic diet. Reduce your salt intake. Reduce your caffeine intake. Avoid spicy and oily food.
