Reducing Hemoglobin ,HDL cholesterol

Dear Sir/Mam I am under medication for diabetes 2 and which is in control.I have some test of blood and urine 3 month before and yesterday the results are Hemoglobin 12.1,11.4,HDL Cholesterol 34,28, Serum creatinine 0.8,0.7 albumin traces in urine both the times.Please suggest what may be the reason and treatment.
