Bipolar Disorder, which you have stated you are suffering from, often manifests with many of the emotional, familial, social and work related symptoms as mentioned by you. This disorder often requires prolonged or at times even lifelong psychiatric and psychological care. In Bipolar Disorder, the sufferer's higher awareness of the mental health conditions it poses can help a lot in its management. With these as the background, I would suggest you to:
1. Be regular with treatment from a psychiatrist, including the uninterrupted intake of medicines as advised by the doctor.
2. Get yourself treated well, uninterrupted, for your physical health issues, through an experienced general medicine specialist doctor.
3. Remain in touch with a psychologist, uninterrupted, to help you deal with your mood swings, family issues, marital relationship issues, sexual issues, trust issues, impulsivity etc. Your wife too may need support from a psychologist.
4. Keep your awareness of Bipolar Disorder up to date so that you can make out your ups and downs in moods and deal with them in an objective way. There are communities of sufferers of this mental health condition about which you will get information from the internet.
Many of what I have written above, if followed sincerely, are likely to improve the quality of life for yourself as well as those who are closely connected with you, like your wife. Hope you will follow them and make life beautiful for yourself as well as for the closely connected ones in your life.
