Hi There are very limited treatment options for Primary myelofibrosis apart from Jakafi.Even with Jakafi tolerance remains a major concern. Therapy responses to thalidomide and lenalidomide remain limited. In case JAKAFI is not tolerated there are options for dose modification, hope all options explored before stopping JAKAFI. At this present clinical status as per your description, we need to have a realistic approach to treatment decisions, with a goal to limit symptoms and discomfort ...sorry  I could not contribute more.. Best wishes Dr Rakesh Pinninti
