Dear Sir, Could you please suggest what should I do? My father, Age 72 years, on steroids for ILD, Dilzem 90 Serum albumin: 2.35 (ref 3.5 -5) Is pedal oedema due to low albumin or Dilzem or any other reasons? Should albumin be supplied to treat oedema? Please see reports attached.

Pedal oedema may be due to low albumin (2.35), but steroids, Dilzem, heart strain (due to ILD), kidney issues, or poor nutrition can also contribute. It would be helpful to check urine protein, kidney function, liver function, and consider echocardiography if not already done.

Your dads age is 72 years. He could have multiple causes of pedal edema apart from low albumin which is also contributing. He needs further evaluation, history. Physical examination and tests. Kindly consult to find the cause and get proper treatment.

The bilateral pedal edema in this patient is most likely related to low serum albumin (2.35 g/dL). Additional contributing factors may include the patient’s chronic illness (interstitial lung disease) and long-term steroid therapy, which can affect protein metabolism and nutritional status. Diltiazem may also contribute to peripheral edema in some patients.
Next Steps
Consult a General Physician / Internal Medicine specialist for initial evaluation. Depending on the clinical findings, referral to a Pulmonologist (given the history of ILD) or Cardiologist may be advised to rule out pulmonary hypertension or cardiac causes of edema. Possible management approach (to be decided by the treating physician): Evaluation for the cause of low albumin (nutritional status, protein loss, chronic inflammation). Dietary protein optimization if nutritional deficiency is suspected. Review of current medications, including diltiazem, if drug-related edema is suspected. Diuretics may be prescribed if clinically indicated for symptomatic edema. Assessment of cardiac function (e.g., echocardiography) if heart-related causes are suspected.
