Sir you are suffering from chronic balanoposthitis which usually results from a persisting fungal infection. Presently the skin glands have been destroyed by the fungus and repair is taking place by scar formation which is not as elastic or natural as the skin. Therefore most of such patients usually keep having the tears again and again despite applying antifungal and many of my patients need circumcision for permanent cure and a healthy sexual life.
