Painfull sperm or urine

When sperm out from penis some sperm stayed inside the tube and very pain in penis. Same case when urine comes doing urine culture test its normal , RE&ME test this result is also normal, doing UROFLOWMETRY its result normal.Doing USG&KUB PVR test here only PVR is109cc. My question is this PVRU is the cause of painful urination or sperm out. Visit urologist and given alfuzosin 10 taking 6days but no result doctor given 30days medicine
