Pain while passing stool

My daughter has fear of passing still since she had constipation a few months ago. She has been on Laxolite for more than a month now. She weights 16.5 kg and takes 15 ml of laxolite every night. She passes stool every other day since she still is scared and holds the stool in. She would cry with pain two- three hours after passing astool saying her anal area is hurting. We took her to a paediatrician two weeks back. Paediatrician saw an anal fissure/ ulcer and prescribed lignocaine hydrochloride gel to be smeared before she passes stool. It has been two weeks now and since last Friday it had become better so I believe the fissure had healed. Today and yesterday while passing stool she screamed of pain. I had already applied the gel on her anal area but she still was in pain. The stool was pencil thin.

As she is struggling since long should be examined fully and also medicine is not working . Should plan for surgery which is a minor procedure with immediate results

Hello, Due to anal fissure and tight anal sphincter she is experiencing pain. And she may have hard stool initially. So along with local application of ointment you can give sitz bath after each stool and also twice a day.
