As u said swelling and pain - did she fell down ? Heavy object liifting or forceful turning of tap or non?
As you said Nerve and surrounding black - mean her hand small blood vessel(capillaries) bleed on pressure so it turn black
Why infection and why pus any injury ?
As she is diabetic , pain can be due to peripheral neuropathy ( nerve of hand and feet get inflammed - lead to pain )
Get her ,
- Cbc , ESR ,
CRP - to rule out infection

Vitamin d , vit
b12 and
calcium level check .
- Sr
creatinine and electrolyte level checked
2nd) XRAY for left wrist with hand Ap view and lateral view
3rd) EMG / NCV test for both upper limb - this to test her nerves are working well any obstruction or compression is there on this nerves
Next Steps
visit a physician for further management if all above mentioned report turn to be normal

1.keep her arm elevated over pillow .it should be above hear level.
2. apply ice to it. (do not put ice direct on skin , put in thin cloth and then applly it)
