Hi,  due to improper brushing the plaque gradually convert into calculus.   Over the period of time This calculus stick on to the tooth,  which lead to accumulation of deposit.  Scaling is the 1st line of treatment follow by deep scaling and root planing if the deposit gone below gum line.
