Need help him get rid of alcohol

He is abusing alcohol since last 15 years. He works as a bus conductor and night shifts only, now age 56. His ECG levels  going down gradually. Sugar level also increasing. Our family is tired of his alcoholism and dipsomania. Please help how to treat him medicaly. Thanks
