Nail uproot

I met with a small accident about 3 months ago where a bike's front tyre jammed into my left top denting it and creating a blood clot underneath it and making one side of the toe growing into the skin. My new nail grew at a different depth and yesterday the old nail that had grown a little just came off and the new nail is underneath and sensitive and as vain as it sound my foot / nail look ugly Do I need a physical doctors appointment or can I keep it covered and let it grow and heal naturally?
