However, since it seems you are quite a distance away, please take steps to consult a Neurosurgeon locally as you certainly need an MRI of the brain and possibly some other imaging before a decision can be made whether you need surgery for this (going only by the report you have uploaded, an operation to prevent neurological problems might be necessary).
