My bay is 1 month old. He has moderate sized VSD. Will it close by itself? Or require surgery? If so , can it be closed with device closure or need open heart surgery? What is the chance of getting closed by itself

You only mentioned moderate vsd. Chances of Spontaneous closure depends on many things particularly the site of vsd. There are more chances of Spontaneous closure for muscular VSDs.
Next Steps
Repeat echo at 3 months of age and follow with Pediatric Cardiologist.

If vsd is less thn 2 cm and its muscular then its get closed by itself , no need for further management . For vsd 2-4.5 cm it need echo screening , but if its more thn 5 cm and non muscular then it need closure .

Hi. Size is not the only criteria. His clinical condition is important. If there is no breathlessness chick active gaining weight with no oulmonary hypertension you can wait for it to spontaneoulsy close otherwise need to device closure based on condition may be there
