Hello, I am Dr Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. A fever reaching one hundred and three degrees in a child can be very worrying, especially when it continues for three days. Meftal P can be used for high fever, but the dose must match the child’s weight correctly. Advices 1 Continue giving plenty of fluids like ORS, soups or coconut water to prevent dehydration. 2 Keep sponging with lukewarm water if the fever rises above one hundred and two degrees. 3 Avoid overdosing paracetamol. Maintain at least a six hour gap between doses. What to Monitor 1 Persistent fever above one hundred and three degrees. 2 Vomiting or refusal to eat. 3 Breathing difficulty or severe weakness. Management Your child is 8.5 years old and weighs 28 kg. The safe dose of Meftal P (mefenamic acid) is usually 5 to 7 mg per kg per dose. For 28 kg Safe dose = 140 mg to 200 mg per dose, taken three times a day if needed. Most Meftal P syrups are 50 mg per 5 ml, so the dose becomes • 14 ml to 20 ml per dose If you are using tablet form • One Meftal P 100 mg tablet can be given, and can be repeated after eight hours if required. Important Do not give Meftal P on an empty stomach. Do not mix Meftal P and paracetamol together at the same time. You may alternate them if fever is too high but only after proper spacing. If fever is lasting more than three days or reaching one hundred and three degrees repeatedly, your child may need evaluation for viral infection, throat infection or dengue.
