Hi, I need to fill my company medical certificate. The form require verification of my blood pressure, weight, height to confirm my medical fitness. The form needs to be attested by medical professional having registration number and stamp with post risk to or higher than MBBS. I have visited lot of clinics and no one provides such service. Such help will be appreciated. Regards Utkarsh

For issuing any certificate a doctor have to keep record and notify it to CMO and most of doctors don't wish to keep those records , so go to govt hospital and follow their procedure
