Name: Syed Abdul Qader Age : 64 No history of any disease except BP that was under control. Had prolonged allergic dry caugh for past 4 years - happened once in a year stays for 30-45 days. He got fewer with sever body pain during mid April with antibiotics got well in few days. Again after 8-10 days it started with body ache and fever following we have started treatment by taking CT scan. Diagnose with lung fibrosis treated in the hospital for 15 days shifted home continuing treatment and gradually taper down the medication. *Current Situation:* *Patient is on oxygen concentrater with spo2 92-94 on 5 ltr oxygen* *On intermediate Bipap* *Cannot get up by himself* *Cannot sit for more than 5 mins on slant / sitting position position bed, that too for medicines and food* *Change of taste, do not feel like eating anything* *Lost tremendous weight, entire bones are visible* *Breathing from Abdomen*
