Low sperms,low motility and low vitality

Sir/mam I am suffering from low sperm count,motility..after two major gastro surgeries. Earlier I was completely fine and everything was normal before these surgeries. My last surgery was done in august. We are planning a baby for the last six month but my wife is unable to conceive due to my problem. Please suggest.
