Low diastolic pressure in 50s and 40s

I have been regularly monitoring my mother's BP which shows up low DBP (initially 60s, then 50s and now 40s). She is on multiple medications [telsartan 80H, aspirin 75mg, prazosin 5mg, cilacar 5mg (initially 10mg, reduced 2/2 low dbp but still low), rozavel 20mg, metoprolol 50mg, apixaban 5mg] and has a PMH of htn, ckd III, diastoilic chf and dvt. Her recent Cr is 1.6 with eGFR of 52 and ACR of 246 (moderately increased albuminuria). She is also on renal diet. I am concerned about low dbp and c/o postural hypotension [supine bp: 130/60, sitting up BP: 100/50 (after 3mins) ].
