Itching near anus and genital region

I have had a recurring problem of itching in the last 2 years or so,initially it started of my thigh region as a bacterial infection.I was recommended by the doctor to apply Zimig cream, along with Candid powder,it was totally fine for about 1 months and then started again,I ignored it for some time and unfortunately it spread to my genital area, including the tip of my penis which gives me a burning sensation while urinating near the tip and my anus feels itchy as well. I want to get rid of this,so please help me doctors.
