Inguinal hernia exam question

Today I had my physical and the nurse practitioner checked me for a hiatal hernia. She did the typical look to the side and cough in both directions but made me return my head to the left again. She claimed she thought she felt something but claimed that it is just a ligament. I want to know if this is a normal thing during this exam or if I need to go elsewhere for a second opinion?

If you have any swelling  in inguinal region while coughing you should consult a surgeon
