Long lists of things to do to overcome procrastination are available on the internet. You will get them from many other sources too. Yet putting them to practice, which has to happen from within you, may not be easy. Continuity, commitment, consistency, determination etc. are things that can be activated by yourself. Towards this you may seek help from a psychologist who will be able to provide you guidance and monitor progress. In the meantime, get your physical health checked by your family physician. Issues like hypothyroidism too make people lethargic. An introspection on if your close family members contributed in your present condition by over-providing may be worthwhile. Communicating more with people close to you, getting involved with things happening around in some situation-appropriate ways, developing some active hobbies etc. may also be helpful.
