When Pcm is ineffective, u can use tab Meftal forte. Also since the fever is high grade, I would advise Fever Profile to be done and follow accordingly. Also do cold sponging, Bes rest, plenty of fluids.

Hello there. I can understand your concern. Since fever is of such high grade, it shouldn't be managed at home . Kindly get a CBC, urine complete, widal and malaria done and consult a physician in person for further help. Ask her to drink lots of water, take a soft diet and have fruits and fruit juices.
