Having a wound in nipple area for 4 mnth

A wound occurred because of itching in my breast nipple area . It produce some fluid it's not healing it's been 4 to 6 months. Some time I feel like it's healing but after some days it start producing the fluid again. The wound is in a circular shape around my nipples there is wound in my nipple also
