Even if only pain on and off. There can be dreadful complications of gall stones like pancreatitis and main duct obstruction which can cause jaundice. Treatment then chances and may involve bigger tests and possibly ERCP and prolonged admission and surgery at later date. It's best to avoid these complications and hence go for surgery. There's no medical management for gall stones. Which can dissolve the stone. The basic pathology or problem is the gall bladder is not functioning properly that's why stone formation.
