Portal vein is normal in course and caliber.
Gallbladder: - Distended with thick tumefactive sludge and wall thickening. No sizeable calculus. CBD is dilated and thick walled with two hyperdense foci of size approx 12 and 13 mm at mid part-? Choledocholithiasis with Cholangitis.
Pancreas: - Normal in size, shape and attenuation. No sizeable mass lesion. No calcification noted.

Consult a gastroenterologist / Surgical Gastroenterologist / surgeon at the earliest. Get admitted immediately. This problem requires treatment. Treatment may be ERCP followed by surgery or direct surgery depending upon the consulting doctors evaluation
