My father suffered from fever for 4-5 times in past 3 months and there was no viral fever or anything when checked And 2weeks back when checked for jaundice and total bilirubin was 10 followed strict diet later got checked for bilirubin decreased to 2 and have loosen up diet the bilirubin value has gone up for 5 today So have taken Ultra sound abdomen test today then gallbladder sludge was formed what step to be taken next? Remedy- medication Any advice or another choice attaching the reports as well Report of HBS,RVD,HCV is NON REACTIVE MP(ICT) - NEGATIVE

MRCP for diagnosis then ERCP stenting for CBD stricture or stone followed by laparoscopic cholecystectomy

Hello! Thanks for sharing your query. This clearly was a case of obstructive jaundice and the cause of which remains unknown till now. He does have risk factor for it, which is the sludge/stone in the gallbladder, however, it needs great deal of attention to rule out other causes. The ultrasound does mentions dilated CBD and a possibility of stricture, but again, it isn’t the best test to prove so.

Detailed history, clinical examination and further evaluation. He may need to undergo MRCP

It looks like a stricture in CBD Problem ur father facing is called cholangitis So is the reason for fever and jaundice He need immediate attention of a gastroenterologist He should be on antibiotics Get an ERCP done for a clear picture of CBD dilation as it is the cause for all the problems Dr sharathchendra Gorantla will be a good choice
