Severe Pain the last 3 balls kind structure below the fingers in the foot, there from 1 month , tried ice pack, heat pad, pain relief spray, all these temporarily reduce pain but then it repeats again in some time

Hello dear Balls in foot that you have described are similar to knuckles in hand and pain over them is called metatarsalgia. Difficult to ascertain the cause without having a look; need to see your footwear as well.
Next Steps
Consult online with clinical pictures of your foot from all angles (let someone else click them) alongwith your footwear. Will advice xrays after seeing them

Do not walk barefoot. No heels

There is a possibility of local skin hardening or soft tissue sprain in that region may be due to continuous work or standing for a long time.
Next Steps
You should get one x ray done to rule out any bony pathology. start hot fomentation,  analgesic tablet.

Active toe movements and use of soft sole footwear can be helpful..
