Possible Causes are
1. Gout
Recurrent pain, redness, and swelling of the big toe joint
Can be mistaken for mechanical joint pain
More likely if dietary factors or family history
2. Hallux Rigidus (Early Osteoarthritis of the Big Toe Joint)
Pain at the base of the big toe (1st MTP joint)
Stiffness, pain with bending the toe, and possibly a bony bump
Pain worsens with walking, running, or toe dorsiflexion
3. Posterior Tibial Tendon Dysfunction (PTTD)
Pain behind the medial side of the ankle and arch, may radiate toward the toe
Common in flat-footed individuals
4. Sesamoiditis
Inflammation or stress injury to the sesamoid bones under the big toe
Pain at the ball of the foot, especially during walking or pushing off
Often seen in runners, dancers, or those wearing high heels or tight shoes
5. Neuroma or Nerve Entrapment
Burning, tingling, or sharp pain
Can radiate from midfoot toward the toe
