Hello, Seems like a case of fistula in ano. Ultimately you will need to visit a surgeon for the fistula, it could range anything from simply laying open the tract to complete removal or laser therapy, although for your case complete removal of tract should be the ideal procedure.

Your fistula needs intervention. Surgical. That's for sure. The bloating and gas formation can be due to 2 reasons. 1. Either because of fistula , you are not passing stools properly and holding in because of pain. 2. Can be underlying another pathology, which can be mostly acid peptic disease or cholelithiasis etc. So need more evaluation. Contact for more info

fistula in ano is an abnormal connection between the bowel (inside of anus) and the skin ( usually near to anus). So medical management is used only for pain and infection there. The definitive treatment for this is surgery/ Laser techniques. So you need surgery for this. Go and consult nearest General surgeon/ proctologist
