Recurrent erectile dysfunction can have multiple causes, including performance anxiety/stress,diabetes,hypertension, obesity,dyslipidemia,smoking/alcohol,medication effects, hormonal problems and vascular disease. Since you previously improved with treatment and symptoms have recurred, reassessment is advisable rather than simply repeating the previous treatment. For proper diagnose need you detailed sexual and medical history. If you want please provide me such information.

Dear Practo User, Let me explain you about your issue in brief - Often frequent masturbation leads to erection problems and penis shrinkage. It seems to me you have been indulged in doing masturbation and watching porn alot. Does it happen you get little Erection you feel you should go in and the moment you try to enter it goes down and it’s not hard enough for you to penetrate? Sometimes even if you penetrate you seem to lose your Erection soon? At some point in a man’s life, he may have difficulty achieving or maintaining an erection. Erection problems occur when you can’t achieve or maintain an erection that’s firm enough to have sexual intercourse. Erection problems are also known as: * erectile dysfunction (ED) * impotence * sexual dysfunction In your case I believe you have developed symptoms of Erectile dysfunction. However this issue is not a major problem to worry as there are medications and treatment available for the same. Rest assured your problem can be resolved. You can contact me directly through my website for online or in person consultation - https://www.drfayyazsexologyclinic.com

ED with urethral stricture is common — the stricture and self-catheterization can affect erectile function through neurological and psychological factors. Since you responded to treatment previously, the same approach should work again.
Next Steps
Consult a urologist for reassessment of your stricture and ED — PDE5 inhibitors (sildenafil/tadalafil) are typically effective. Addressing psychological stress also helps. Consult me on Practo for further guidance.

First, please be reassured: the fact that your erectile dysfunction previously resolved completely with treatment proves that your vascular and nerve pathways are capable of generating strong, normal erections. Recurrent erectile difficulties at age 42 are very common and typically stem from one of two interacting pathways: Performance Anxiety & Psychological Conditioning: When a man experiences even a single episode of difficulty, it often creates anticipatory stress during future attempts. Worrying about maintaining an erection releases adrenaline (epinephrine), which directly constricts penile blood vessels and causes the erection to subside immediately. Underlying Vascular or Metabolic Changes: At 42, early vascular or metabolic shifts (such as subtle fluctuations in blood pressure, blood sugar, lipid levels, or testosterone) can reduce endothelial nitric oxide production, making erections more sensitive to stress or fatigue. Lifestyle & Stress Factors: Chronic workplace stress, poor sleep quality, or lack of physical activity can lower baseline energy and sexual responsiveness.
Next Steps
Consult a Urologist, Andrologist, or Sexologist: A structured clinical follow-up will help review what previous treatment worked for you and determine if a short-term course of medication is needed to restore your confidence. Undergo Essential Laboratory Screening: Hormonal Profile: Early-morning Serum Total Testosterone, Free Testosterone, and Prolactin. Cardiometabolic Workup: Fasting Blood Sugar, HbA1c, and Lipid Profile to ensure optimal vascular health. Blood Pressure & Cardiovascular Check: Ensuring normal endothelial and cardiovascular function. Evidence-Based Medical Management (Under Prescription): PDE5 Inhibitors (e.g., Tadalafil or Sildenafil): Your doctor may evaluate prescribing an on-demand or low-dose daily PDE5 inhibitor. Daily low-dose Tadalafil (5mg), for example, provides steady vascular support and takes away the pressure of "timing" medication around sex. Psychosexual Counseling / Sensate Focus: Engaging in sex therapy or mindfulness-based intimate techniques helps eliminate the spectatoring habit (watching yourself perform) and lowers adrenaline release during intercourse.
Health Tips
Absolute Word of Caution: Masturbation does NOT cause "penis shrinkage" or permanent organic erectile dysfunction. Ignore unscientific myths that attribute sexual issues to past masturbation habits; such claims only generate false guilt and intensify performance anxiety. Avoid Unverified Over-the-Counter Supplements: Do not buy unregulated herbal stamina pills or unprescribed stimulants, as they can contain hidden contaminants and carry cardiovascular risks. Optimize Cardiovascular Health: Regular aerobic exercise (such as brisk walking or jogging for 30 minutes daily), pelvic floor exercises, adequate sleep, and reducing alcohol consumption naturally improve penile blood flow and nitric oxide availability.

Since you have experienced erection difficulty previously, improved after treatment, and are now experiencing it again, this is a recurrent erectile-function concern and should not automatically be assumed to be permanent ED. Difficulty maintaining an erection can have physical, hormonal, medication-related, lifestyle or psychological/performance-related contributors. A proper medical, sexual and psychosocial assessment is recommended to identify the reason for recurrence.
Next Steps
Please avoid restarting previous ED treatment on your own without understanding why the symptoms have returned. A consultation can assess your current erection pattern, including whether you have normal erections during masturbation or sleep, and determine whether further evaluation is required. If performance anxiety is contributing, addressing it can be an important part of treatment as well. For better treatment and personalised guidance, you may connect with us at 9 3 1 5 6 6 1 5 6 5 for proper assessment and guidance.
Health Tips
Maintain regular physical activity, adequate sleep, a healthy weight and good stress management. Avoid excessive alcohol, smoking and recreational substances. During intercourse, try not to continuously monitor the firmness of your erection, as performance pressure can itself cause an erection to fade. Recurrent erection difficulties are common and treatable, so there is no need to panic or assume that the previous treatment has permanently stopped working. Thank you
