Dull pain in left testicle

I am having this dull pain in my left testicle for quite some time. The pain sometimes increases upon wearing tight underwear for. Scrotal support. My Scrotal USG is normal. I have attached it here. Can you tell me why I have this pain and when will it go away

If ultrasound is normal then then this pain can be due to tight underwear or may be neurogenic origin. Need to consult for further evaluation and treatment.
