Do I need to extract this tooth?

I am dealing with one tooth issue for 1.5 years. No relief yet.My pain started with lower jaw pain under ear lobe,severe ear pain and mild swelling in the lower jaw.Went to dentist to check for jaw pain under ear,he spotted tooth 46 issue.I didn't go in with tooth pain. 46 hurts for tapping and biting pressure. Clear xray and cbct. Suspected root fracture.Sent me to Endo. He didn't find any fracture.46was little sensitive for cold.tapping and biting pressure pain on it. So told to get root canal. Attaching x-rays before and after RCT.Had RCT Sept 2020. No changes in the pain even after root canal.It's the same tapping and biting pressure pain. No changes at all after Rct.Taken another cbct and nothing found.Checked for TMJD and normal MRI, CT. Endo couldn't find what's the issue and wanted to retreat the tooth RCT to see if it helps &said it's a guess. Went to 2nd Endo and he said some inflammation present marked in green circle. Told to pull it out. Do I need to pull or redo Rct?

The green marked area seems like fracture, go for cbct for confirmation

Hi Did the pain specifically start behind the ear lobe ? Root canal seems fine. If you could share your symptoms in details since sept 2020 it would be easier to figure out what is wrong . It may not be pertaining to just teeth . Please get an OPG X-ray made
