Dizzy, muscle and joint pain, brainfog

I have dizziness,knee shoulder joint lain, back pain radiating to legs muscles, forgetfulness, lack of presence of mind and mood changes since 2 months before which i had gerd so i was orescribed PanD for 3 months. I had my CBC done in which my Mchc (28.8), high RDW (14,90) and little high lymphocytes (3.21, normal is 1-3) but normal hemoglobin (14.2) and rbc(5) and other factors are also normal totally. Ehat could be possible reason? Could B12 or B6 or vitamin D be the reason? Please help me out because I am also facing bit of lack of breathe since 3 days. What could be the possible reasons? Attached my report here...
